Travel Voucher Forms

The Controller’s Office has released new travel voucher reimbursement forms for all domestic travel and extended, domestic travel.  They have not updated the vouchers for international travel yet, but those should be forthcoming within the next few months.  The new vouchers have been posted out on the Controller’s Office website along with instructions for use.  They are located at:  https://louisville.edu/finance/controller/acctops.  The forms can also be found on the “Travel” page and the “Forms” page.

ULINK

ULink is the site for internal news and information and a point of access for employees and students to conduct university business.  http://ulink.louisville.edu

Qualtrics

Faculty, staff and students are entitled to a Qualtrics Survey Software account.  Qualtrics is an on-line survey software.  The link is http://qualtrics.com/academic-solutions/university-of-louisville/

Authority

Authority is a web-based system that makes it easy for faculty to keep track of the activities they accomplish, such as the teaching, research and service information found on their CVs.   Authority is now available from off-campus computers. Use your UL credentials (username01) and ULink password. At any time you have a problem getting in, etc. select the “Report an Issue” at the top right of the Authority page.

Research & Publications

On our website, we have a separate page highlighting faculty research accomplishments (http://business.louisville.edu/research-and-publications).  Please report your research accomplishments in the format on this form.  Once completed, send the form to Cindy McDonald at cindymcdonald@louisville.edu.

Online Course Evaluations

In 2011, the university moved from a paper-based evaluation system to https://blueeval.louisville.edu/ce. Students can go to this site to access evaluations that they need to fill out, and faculty can access reports of their completed course evaluations here as well. Students can also access their evaluations through BlackBoard. You can log into the site using the same username/password that you would use to log into ULink.

College of Business Classroom Technology and Assistance

The College of Business Information Services department is responsible for the computers and a/v equipment in the classrooms and faculty/staff offices. Should there be a problem with any equipment, please call the COB HELPDESK at (502) 852-0154 or email cobhelp@louisville.edu.

Staff Development Application

The application is in Adobe PDF. Complete the form, print, sign, get supervisor’s signature, then submit to Jane Goldstein.

Funds are not available to pay tuition for course academic credit, books for an academic credit course, trips that are a component of an academic degree program, etc.  A staff member may not apply for staff development funds during Provisional Period (first six months of employment in the College of Business).  Applications will be accepted after deadline depending on available funds.  New Deadline:  July 12, 2013.  COB Staff Development Application Form 2013-2014.

Teaching & Request A/V Equipment for Davidson Hall

Some of the College of Business classes are taught in Davidson Hall classrooms. The College of Business does not provide audio/visual or computers to these rooms.  They must be requested by the professor through their IT Support media request: -http://louisville.edu/it/help/classroom/media-belknap; phone is 852-6465.
